Your local Coram dietitian, pharmacist and nurse works together with you and your family to make sure you are fully educated and comfortable with receiving nutrition therapy in your home. We are dedicated to providing high quality clinical care and personal support for nutrition consumers. Specific program components include hospital discharge coordination, primary and ongoing consumer and family education, a personal nutrition and clinical support team, monitoring of nutrition needs by a registered dietitian, disease-specific therapy management, 24/7 availability, and personal travel support.
Intradialytic Parenteral Nutrition (IDPN)
Coram's Nourish Program also provides individualized renal nutrition products and services to dialysis centers and chronic dialysis patients who are malnourished. Recognizing the clinical and economic impact of an optimized nutritional status, the Coram philosophy is to partner with both patients and members of their care team to meet each patients' nutritional needs. Coram's team of nutrition support specialists (dietitians, pharmacists and nurses) can provide clinical support in identifying appropriate candidates for IDPN, ordering IDPN and monitoring the response to therapy.
